Registration and Helpful Forms

If you're ready to officially register your child for the coming school year, please enter your email and select your registration type below to receive a registration packet for your child. The packet will be sent to the email address you provide. You will notice that the forms are divided into several sections.

If you'd like to complete and sign the forms electronically, you'll need the free Adobe Acrobat Reader. Save the packet from your email to your device, open it in Adobe Acrobat Reader (not your web browser), and fill in the fields. To sign, use the Fill & Sign tool to type, draw, or upload your signature — no "digital ID" or certificate is required. Once complete, save the packet and email it back to caos@carle.com.

If you would prefer to print and complete the forms but don't have access to a printer, please contact us with your mailing address and a packet will be mailed home to you. If you do have access to a printer, please print, complete, and return the registration packet using one of the following options: scan and email the packet to caos@carle.com, fax the packet to (217) 326-2266, or mail the packet to:
